If so, this is an exciting opportunity for a Practice Manager to join the Pathways Leaving Care Service, responsible for delivering statutory services to young people aged 18+ who have left care.

The service currently consists of two pods, each led by a Practice Manager who provide support to a team of Pathways (Personal) Advisers. You will also work closely with colleagues across the council and partner agencies to ensure our care-experienced young people receive the support they need to thrive and achieve positive outcomes.
We are looking to recruit a Practice Manager to supervise and support staff within the Pathways Leaving Care Service. The role of the Practice Manager will be to supervise and lead a team of up to 6 Pathway Advisors and social workers / student social workers as and when needed.
The Pathways Service supports young people leaving care to successfully transition into independent living and adulthood. Calderdale Childrens Services is a systemic organisation and as such, it operates a Pod model, alongside regular training and support for staff including pod reflect meetings with the team.

We are seeking someone to join the dedicated team who has a passion and desire to continuously improve outcomes for care-experienced young people transitioning to leaving care and independence. The successful candidate will be able to demonstrate that they can support the service to achieve its goal of becoming an Outstanding service.
Applications will be considered from candidates wishing to progress into management or current managers looking for a development opportunity within the Pathways Service.
Applications will only be considered from those who hold a recognised social work qualification in a relevant field and can demonstrate a sound understanding of the Legislation in respect of children leaving care and the care planning regulations and guidance.
