Calderdale Council is the most improved adult social services Directorate in Yorkshire Humber. Our work has been nationally recognised winning a series of awards in recent years. We currently have an opportunity for a Social Worker to join within the Intermediate Care Social Work Team. The Intermediate Care social work team offers a service to people whom have received ongoing rehabilitation following hospital discharge in Halifax and Huddersfield. These roles involve close liaison with health colleagues, to ensure a good discharge from Intermediate Care settings.
The Hospital and Intermediate Care team work tirelessly with people, their families, carers and other professionals to uphold human rights.
Social Workers receive an extensive Continuous Professional Development offer.
