Our work has been nationally recognised winning a series of awards in recent years.  We currently have an opportunity for a Social Worker to join the Hospital and Intermediate Care Social Work Team.

 

Over the last 6 months the team have undergone significant change, during a period of exceptional challenge presented by COVID. The hospital and intermediate care setting is a very fast pace working environment, operating rapid discharge through the implementation of a discharge to assess model. We are excited to be able to offer this opportunity to an enthusiastic, resilient and dedicated social worker.

 

You will join a supportive team who are responsible for undertaking assessments to help facilitate good discharges from Calderdale Royal Hospital, Huddersfield Royal Infirmary and intermediate care settings. The role involves close liaison with health colleagues, other professionals and organisations to promote health and wellbeing.

 

The team are strong advocates of the Mental Capacity Act and Human Rights Act,

ensuring that the human rights of individuals are at the forefront of service delivery in these unprecedented times. The team works in genuine partnership with people, their families and carers.

 

Social Workers receive an extensive Continuous Professional Development offer and the team promote a positive learning environment.
