An experienced Social Worker is required to join the Disabled Children’s Service, Inclusion and Access, West. You will hold and manage a caseload of children and young people across the whole age range 0-25 as allocated by the Locality Team Leader. You will assess, co-ordinate, and review placement arrangements locally for children and young people with SEND who are subject to the Children Act 1989, Children and Families Act and Care Act. This work also operates within the context of the new model for preparation for adulthood by ensuring young people are effectively supported through the 0-25 age range. This role will include negotiating financial contributions to packages of support and working in partnership with parents, support planners and other professionals in order to ensure children and young people have opportunities for community activities and short breaks and ultimately make a successful transition to adulthood. There will be a requirement to manage Safeguarding and Looked after Children casework, dependent on knowledge and experience.

The post holder will need to have a commitment to shared values and the common purpose of developing a culture of interagency working; including statutory bodies, third and private sector organisations.
