A vacancy has arisen in the North Ryedale Planned Care Assessment Team for a full time Social Care Assessors Social Worker
The current office base is at Ryedale House in Malton and the Council is committed to enabling staff to work more effectively, including working from home on occasions. You will need to be able to demonstrate knowledge of assessment experience and have a Social Work Qualification. You will be working with a range of people in the area accessing care and support at home, community activities and daytime support via contracted support and use of Direct Payments. This role includes working with adults with learning disabilities, physical disabilities and older adults. The Planned Care team works with adults over the age of 18 living in the Northern half of Ryedale District including the market towns of Helmsley, Kirkbymoorside, Pickering and Thornton le Dale.
You will support people living independently, with family, in adult placements, supported living and residential or nursing homes. You will be expected to work creatively with people so as to fully utilise their skills and life opportunities, whilst recognising the current budgetary restraints for Health and Adult Services. You will need to be aware of, and be able to work within the legislative framework of The Care Act and Mental Capacity Act. You will need to be able to communicate effectively with a range of people requiring support, their family carers and other professionals, especially at time of hospital discharges.
You will be supported to develop your skills and knowledge so as to undertake safeguarding enquiry investigation and Best Interests Assessments at the appropriate stage of your professional development.
