We have an opportunity for a Social Worker (Social Care Assessor) to fill a full time or part time established post in our Independence and Reablement team.

The team are based in Richmondshire and covers the Dales patch and bordering up to Darlington.

We carry out assessments in all the local hospitals within this area including; James Cook University Hospital, The Friarage, The Friary, Darlington and Bishop Auckland Hospital.

As a Social Care Assessor or Social Worker within the Independence team we carry out assessments for referrals from; hospitals, community health teams and individuals and their families. This role work is of a very fast pace and involves short term intervention and prevention work, long term interventions are completed by the Planned care Teams.

This ensures that we observe all opportunities of prevent, reduce and delay the need for long term social care support. You will undertake a range of asset strength based assessments and interventions to ensure that adults with care and support needs and their carers achieve their desired outcomes and wellbeing to live independent lives.

This will also include developing personalised care and support plans with the person and the reablement team that represent best value. You will ensure that the person is safe from harm but where harm or abuse is identified ensure the safety of the person and undertake the relevant safeguarding enquiries.
