Are you looking to be part of an innovative, committed, diverse and supportive Social Care Team?
These posts are part of a new service structure providing an excellent opportunity to become part of the team embedding social care within adult mental health and are available from 1 May 2019.
As a Mental Health Social Worker you will work closely in partnership with a variety of teams and agencies, including mental health colleagues, prevention teams, voluntary and independent sector, health and public health colleagues. Our ethos is to promote health and wellbeing, preventing, reducing and delaying need, in line with the Care Act.
You will undertake a range of asset and strength based assessments and interventions to ensure that adults with care and support needs and their carers achieve their desired outcomes and wellbeing to live independent lives. This will also include developing personalised support plans that represent best value. You will determine eligibility for adults with care and support needs against the national standards. You will act as Safeguarding Enquiry Officer, undertake Mental Capacity Assessment and Best Interests decision making and be responsible for complex case management. There may also be the opportunity for you to train as an Approved Mental Health Professional as posts become available through the service.
We welcome applications from newly qualified Social Workers and offer support through ASYE.
Candidates must be highly focussed, committed and adaptable individuals who have an aptitude to work in a fast paced environment and have good organisational and communication skills.
If you would like to join our great team, we would like to hear from you!
