Wakefield Council has an exciting opportunity in the Connecting Care West team for a full time, permanent, level 1 Social Worker.

 

The successful candidate will join an experienced and supportive team of Social Workers and Care Coordinators who work with our service users in the West of the district. Connecting Care West support adults over the age of 18, who have care and support needs.

 

We are looking for highly motivated, committed and enthusiastic individual with drive and passion who seek to achieve the best outcomes for the individuals that they work with, promoting and enabling strengths based approach.

 

The successful candidate will be responsible for their case load which will include completing strength based Care Act assessments to identify needs and the development of outcome focused care and support plans. Connecting Care West are seeking a Social Worker who has the ability to prioritise work, be outcome focused, and an excellent communicator and team player.
